Healthcare Resources for Survivors Patient Information Cards We are offering trauma-informed information cards to patients, produced as part of the #CheckWithMeFirst campaign from The Survivors Trust. These cards can be downloaded to be printed off or saved to a phone for survivors to take into their appointments to help inform their healthcare practitioner that they require some small adjustments to their care and a trauma-informed approach. We hope that these cards will help to empower survivors and make appointments that are potentially anxiety inducing, a little more comfortable. Alongside making these available on our website, we have also handed these out to NHS Champions who have completed our workshops to use in their workplace as part of a trialing period for these cards. Non-branded Trauma Information Cards We offer non-branded cards which do not mention sexual abuse or assault for survivors who do not wish to disclose that or do not feel comfortable doing so. We have two types of non-branded patient information cards. The smaller ones which give general information and advice to healthcare practitioners are available here, and larger ones which provide additional space for survivors to specify these adjustments in more detail are available here. The Survivors Trust Branded Trauma Information Cards We also offer The Survivors Trust-branded cards. We have two types of branded cards. The smaller ones which give general information and advice for healthcare practitioners are available here, and the larger cards which provide additional space for survivors to specify these adjustments and go into more detail, if they wish to, are available here.
